From what I remember of California they do a sniff of the exhaust of the car at idle and at 2000RPM and there is a visual inspection for anything that could possibly foul up the emissions system of the car or change in a negative way the emissions. (Carb OE#s are your friend out here) They also check that the car idles below 1000RPM and has a working CE light.
If you get sent to the Ref in California then you have to make sure that your car's engine and exhaust system are up to the manufactures specs for that engine. Also if you have bolt-ons you need to have the CARB OE#s in the engine bay, near the bolt-on part. Then they strap your car down to a dyno and run the car under simulated load while doing the sniff test. Any visible bolt-on power adder that you don't have a CARB OE# for will fail you instantly and they do a through inspection from what I understand.
I also recall reading that it was easier to pass the smog tests and ref tests if you drove the car around pretty hard right before the test to heat the cat and header up.
That's what I remember of it. I might be totally smoking crack though.
